To travel with a psychiatric service dog, United Airlines requires passengers to submit the following forms:

• Department of Transportation (DOT) Service Animal Air Transportation Form
• Service Animal Relief Attestation Form (for flights over 8 hours)
• Psychiatric Service Dog (PSD) Letter from a licensed mental health professional, confirming the need for a PSD.

You have two options for adding a service animal to your travel with United Airlines:

• When booking your ticket, check the service animal option when entering your traveler information.
• After booking your ticket, select the option to add a service animal when viewing your trip.

You need to call the Airlines for

• International flights
• Hawaii flights
• Guam flights

• Fee: $125 each way.
• Pets allowed in the cabin: Dogs and cats only.
• Carrier requirements: Must fit under the seat and remain secured.

When flying abroad, you have to present rabies and health certifications in accordance with United Airlines’ pet policy. The vaccines against rabies must be finished at least 28 days prior to travel to the United States.
You need to have a pet health certificate with the following details if you’re going inside the United States:

• Name, phone number, and address.
• Specifics about your animal companion, like breed, color, age, sex, and patterns.
• Names, dates of issuance, and expiration dates of recent vaccines are mentioned.

• Hard-sided carriers: Length 17.5” x Width 12” x Height 9” (44 cm x 30 cm x 22 cm).
• Soft-sided carriers: Length 18” x Width 11” x Height 11” (45 cm x 27 cm x 27 cm).

Your pet must remain inside the carrier, secured under the seat in front of you.
